Rehab at Home Program
Have you or a loved one had a significant decrease in your mobility after a hospitalization or other health event?
Have you been told you cannot not go home after a hospitalization because you need daily physical therapy that you cannot receive in your home?
Would you prefer to go home instead of rehab due to concerns about staying in a rehab facility?

Oftentimes in the hospital, patients and families are told that going to a rehab facility is the only option due mobility difficulties, and that you cannot return home.
You might have been told that standard in-home physical therapy is not enough, as they will only come see you 1-2 times per week, and you need more care.
We are here to offer you another option. With an appropriately guided program- many of these patients CAN safely recover at home.
Our “rehab at home” program offers physical therapy services at the same, if not a greater intensity, than you would receive at rehab, all in your home, where you belong.
The program:
High intensity, short duration PT programs delivered at home, designed to replicate rehab.
1-2 therapy sessions delivered per day, every day, to meet your needs.
All therapy equipment needed is brought to your home- you do not need to provide anything
Same therapist every session, pushing you towards your goals while also providing family and caregiver training to maintain safety during recovery
Phone and text message access to your therapist between sessions to address questions or safety concerns.
2,3 and 4 week options available to meet your goals regardless of your current status.
How it works:
Your therapist will meet you at your home as soon as possible after discharge-typically the same day or the next day and perform an evaluation.
Goals will be set to address safety and patient functioning
Initial safety recommendations will be made as well as equipment recommendations.
Short term, high frequency PT will be initiated immediately- designed to simulate rehab from the comfort of your home.
FAQs:
Is this program covered by insurance?
While some insurances such as traditional Medicare may cover a small portion of this program, the intensity and frequency of this program will not be covered at 100% by any insurer. This means there will be a considerable out of pocket cost, as this is a premium service that is an alternative to an inpatient rehab stay.
Is this home health?
While this program is delivered at home, there are significant differences between and and a typical Home Heath agency. A typical home health offers therapy 1-2x weekly, and we aim to offer much more. In addition, we do not offer nursing or home health aide services.
What if I really need rehab?
That's okay! If you have a medical need such as frequent nursing care, have limited social support, or an unstable medical condition, rehab may be the best option for you. Unsure? Contact us and we can help you decide.
What is the service area for this?
We primarily cover Sussex County and portions of Morris County (Rockaway, Lake Hopatcong areas). If you live outside this area, reach out to us anyway, we may be able to help with an additional travel fee.
